Introduction
Diabetes Mellitus as a Syndrome.
Definition of Syndrome
A group of signs and symptoms that occur together and characterize a particular abnormality or condition. Multiple Causative factors like Genetic Tendency , Environmental Factors , Food habits , Life style , Obesity , Mental Stress , Auto immunity. Miasmatic Causation – As per Homoeopathic Philosophy Sycosis is sole cause of DM Type II
Sycosis And DM Type II
DM Type II is an Auto immune disorder mediated by Auto Antibodies leading to Insulin resistance and reduction in effectivity of action of Insulin on carbohydrate metabolism. Auto immunity is due to defect in preprocessing of B – Lymphocytes. Preprocessing is highly influenced by emotional stress breaking down immune tolerance. Sycosis is tendency of body to be in excess. DM Type II have raised level of Serum Insulin with reduced effectivity.
Need of Anti Miasmatic Remedy
Our study of 229 cases shows that –
• Thuja is indicated in majority of cases( 24 cases) along with Calcareia Carb (26 cases)
• Frequently indicated remedies are having Sycotic Miasam in there drug proving
• Even almost every case need Anti Miasmatic (Anti Sycotic ) remedy either as Constitutional remedy or as an Inter current remedy
Probable Mechanism Of Action Of Homoeopathic Remedies
• Reduction in mental stress.
• Correction in pre processing of B – Lymphocytes
• Reduction in Insulin Resistance - (Easily judge by observing skin for Acanthosis nigricance)
• Correction of Truckle or Central Obesity or Visceral Fat - ( Measured by Waist to Hip ratio).
Homoeopathy As Stand Alone Therapy
• Homoeopathy As a Stand Alone therapy –
1. When case is newly diagnosed with Type II DM
2. End organ damage or complication are not yet occurred.
3. Causative factor is clearly identified.
4. Less significant family history.
5. Waist to Hip ratio not in too excess.
6. Acanthosis Nigricans is not too dark.
Homoeopathy As An Add – On Therapy For Type II DM
• When patient is already on conventional treatment like ORA.
• When patient is already taking Insulin.
• When serious complication and end organ damage already occurred.
• When patient is already under sever sepsis.
• Where surgical interventions demanding urgent glycemic control.
• Strong family history of Diabetic complications.
HBA1C as guide to understand effect of action of Homoeopathic remedy for Type II DM
• HbA1C is glycated Hemoglobin which tells us long term status of Blood Sugar Levels.
• This test is use full in both condition as Stand Alone Homoeopathic Rx & Add- On Homoeopathic Rx
• It is very use full to confirm that selected remedy is Effective or Not Effective.
Potency And Drug Interaction During Homoeopathy As An Add – On Therapy
• Homoeopathic higher potentiesed (Diluted) are highly safe as no any drug interaction is possible with conventional medicine.
• Homoeopathic potency work best when patient is on an Insulin therapy than OHA.
• Mother tinctures may give drug interaction with OHA.
• Mode of action of Homoeopathic potency is different from OHA.
• OHA reduce blood sugar level either by reducing carbohydrate absorption or by increasing insulin sensitivity.
Pathology Of Diabetic Neuropathy
• Common complication of Type II DM case even in strictly controlled as well as uncontrolled blood sugar.
• Symptomatic presentation like burning , numbness , tingling in extremities.
• More common in lower limb due to more length of nerve.
• Easily clinically assess using tuning fork of 526Hz frequency.
• Autonomous nervous system dysfunction showing constitutional symptoms like giddiness , postural hypotension.
Reversal Of Diabetic Neuropathy
• Symptomatic relief is possible with simillimum remedy.
• Easily assessed by using tuning fork or biothesmometer.
• Complication such as diabetic foot can be prevented.
Homoeostasis is Vital Force
• Homeostasis-
Maintenance of a nearly Constant Internal Environment.
• In 1929, the American physiologist Walter Cannon coined the term homeostasis to describe the maintenance of nearly constant conditions in the internal environment.
• Essentially all organs and tissues of the body perform functions that help to maintain these relatively constant conditions.
Hahnemann & Concept of Receptor
Hahnemann's concept -"Similia Similibus Curantur" marks the beginning of the concept of receptor interaction with drugs.
Paul Ehrlich and John Langley - formulated the concept of drug receptors.
• The Sympathetic Nervous System (SNS)was the focus of Langley's work on the reaction of cells and of nerve endings to certain poisons chiefly reaction of striated muscle to nicotine and curare(1905) and the physiological action of ergot on uterine muscle (Henry Dale 1906).
• Langley in 1905 had given reasons for supposing that Curare, like nicotine ,combine with a "receptive substance" of muscles.
A. V. Hill - Investigated the effects of nicotine and curare on contraction of the frog rectus abdominis muscle, by using mathematical and Physico- Chemical Methods.
John Henry Gaddum – Concept of competitive antagonists. The acetylcholine receptor was the first receptor to be studies in details.(Hahnemann would have been thrilled to know this since he used Belladonna for proving).
A. J. Clarke – Quantitative work on the interaction between drugs and receptors. His work on acetyl choline showed that the antagonism between acetylcholine and quaternary ammonium salts could be explained in terms of the drugs competing for a common receptor.
Concept of Receptor Binding
• Hahnemann – The concept of symptoms as a source of both Pathological & Therapeutic diagnosis.
• "Similia Similabis Curanter" Law of similar as a therapeutic approach based on Ligand Receptor Interaction
• Drugs tested by him on himself are all best candidates for receptor panning using today's technology
• Hahnemann conceived the symptomic affinity of drugs for tissues, Symptom similarity of drug and tissues are essential for the treatment of medically curable disease.
Hahnemann & The Concept of Hormesis
Hormesis means to Excite.
In toxicology, hormesis is a dose response phenomenon characterized by a Low Dose Stimulation, high dose inhibition, resulting in either a J-shaped or an inverted U-shaped dose response.
Hormesis explains physiological bases of Hahnemannian method of
Drug Potentization.
Microbiome Role in Chronic Diseases
• Tang & Hezen
Explore how intestinal microbial metabolism exerts
effects on host metabolism including and culminating in Atherosclerotic
,Cardiac Vascular Disease.
• Type II Diabetes patients have a significant reduction in Firmicutes, Clostridia and Bifido bacteria.
